What is Reablement Care?

After someone has been involved in a serious accident or an illness that has meant a prolonged stay in the hospital, it can be difficult for them to readjust to the life they once had at home. This is the same for people with long-term disabilities or significant health conditions. 

Our reablement service, therefore, aims to help these people get back to their everyday lives. We will help our clients to redevelop their skills so that they can once again live independently. 

This includes tasks such as ablutions, washing and dressing. We will also help people get back to routine tasks such as preparing and cooking meals and other general duties around the home. Ultimately this reablement service aims to prepare our clients for an independent future.

  Individually tailored assessments to understand what our clients need once they return home 

  Inspect and prepare our client's homes for their return, including equipment and safety checks 

  Liaising with other medical professionals in the NHS and local authorities or councils

  Ensuring that our client's family members and relevant persons informed and updated of their condition

  Arranging subsequent services, such as physical therapies or medication routines

  Regular reviews and updating of service reports to make sure we are providing the correct treatment

  Other service options when required, such a palliative or dementia care services

Key Features of the Reablement Approach

Our approach to reablement services is many-fold and provides comprehensive care services for those returning from prolonged stays in hospital or suffering long-term conditions. 

Our first goal is to maintain and promote our client's independence in their own lives, ensuring that their wellbeing and mental health is of maximum importance. This helps boost our client's abilities once they return home and aid them in living full and meaningful lives. 

Unlike our other live-in care services, reablement involves our carers taking a back-seat role, encouraging our clients to act for themselves independently. This is done with an understanding, patient and compassionate frame of mind, ensuring that our clients are comfortable with the service they are receiving. 

Our highly-trained and specialist team of carers will care for and support our clients to help them regain their skills so that they will be able to care for themselves once again in the future. 

This means that our reablement services are usually short-term, generally up to six weeks in duration. Our ultimate focus is to restore our client's liberty and independence to effectively manage their own health needs rather than rely on healthcare professionals. 

This includes ensuring that our clients do not need to be readmitted to the hospital and that they can fully return to their lives at home.
